Use a Personal Form

You can choose from any of the Form Templates in your account to complete and submit in Huddles. Depending on the settings you choose for your Personal form, other participants in your Huddle can see your Personal Use form submission. Super Admins and Account Owners will be able to see your form submission, regardless of their role in the Huddle where you submit a Personal Use Form. If you wish to Use a Form in a Goal, you can also do that by adding it to an Action Item when creating a Goal. For more on Forms in Action Items, click here.

Who is this article for?

Based on the default user settings in the Sibme platform, you should have permission to complete this task if you are one of the user levels listed below. If you do not see the screens in this article, you can ask your administrator if you have permission.

  • Users

  • Admin

  • Super Admin

  • Account Owner


Steps to complete this task

  1. Click on the Green Plus button from your Huddle and select “Use Form”

  2. A list of Form Templates will display, you can click the preview button to see the full form.

  3. Click the Use button to begin completing the form (either from the list of forms or from the preview).

  4. If you would like to save your work before you have completed your form, click Save. This will allow you to return to the form later. The Saved form will be listed among your Artifacts.

  5. When you’re ready to submit your form, click the green Submit button.
