Creating a Form Template

In order for people to use Forms in your Sibme Account, you will need to build custom forms in your Account Settings. Form Templates are built in the Form Builder. Click on the forms button in Account Settings to start building your form templates.

Who is this article for?

Based on the default user settings in the Sibme platform, you should have permission to complete this task if you are one of the user levels listed below. If you do not see the screens in this article, you can ask your Sibme account administrator if you have permission.

  • Super Admin

  • Account Owner

NOTE: Once you publish a Form Template, users in your account will be able to use, assign, and submit that form in Huddles and Goals. If you edit the Template after someone has assigned or submitted it, changes in your template will not be reflected in the already-assigned or submitted form.


Steps to Accomplish this Task

  1. In the Form Builder Module, click the Green Plus Button to create a form

  2. Create a Title for your Form. This title will display at the top of each form when it is assigned or used by a user. However, the description will only be visible when you review already created Form Templates in the Form builder and in the list of forms a person can use or assign. The description will not be visible in the form itself.

  3. On the left side of the screen is a menu of modules you can add to your form.

To learn more about special module options click on the links below:

  1. Click and drag the module you wish to use in your form into the box at the center of the screen. Once you drag it into the Form Template, you will see a preview of what the module will look like in the form at the top of the Module, with options to configure each module below the preview.

  1. You can add as many modules to your form as you wish. To collapse a form module after you’ve configured it, click the ^ icon in the bar. To duplicate a module, click the

    icon. To delete a module, click the trash can.

  2. You can reorder your modules after adding them to the form by clicking and holding the horizontal bars and then dragging it up or down to the location you wish for it to be in your Form Template.

  3. You can preview what your form will looks like for Users by clicking the eye icon at the top right of the screen. If you are in preview mode the icon will turn gray. To continue editing your form, click the icon again.

  4. Once you have finished building your template, click the Save button at the bottom of the screen.

  5. After saving, your Template will show on the main screen of the Form Builder in Account Settings, from here you can click the three dots to View, Edit, Rename, Duplicate, Archive, or Delete the Form Template from your account.

    The “Locations” box in each row will allow you to see where your form has been assigned, used, or submitted in Goals and Huddles, and also where you can view each submission.
