You can collect a variety of types of data in Sibme forms. This article describes the various types of form Module Options and links to further details about some of the more unique options available in Sibme Forms.

For modules in your form where data can be entered by users when they complete and submit a form, you will always have the option to make the module required. Other options are listed below.

  • Heading: This module allows you to add formatted (size and color) text that will be visible in your form. Heading text is edited when building the Form Template and will always be visible in the Form.

  • Details: The Details module allows you to create text description and instructions in your forms. Details have rich-text formatting options that will be visible in all forms once the Form Template is available.

  • Horizontal Line: This will insert a line between two modules in the Form. You can choose the thickness and color of the line.

  • Text Field: This allows you to create an opportunity to collect unformatted text from users when they complete and submit a form. You can also enable “Validate as Email” to ensure that the text entered is in the valid email format.

  • Text Area: This allows you to create an opportunity to collect formatted text from users, including paragraphs.

  • Number Field: This will allow users to submit any numerical data. The number field will only accept numbers or the characters . , and $

  • Date Picker: This module will allow users to enter a date in the MM/DD/YYYY format or choose a date from the calendar module that appears.

  • Checkbox: This module allows users to select as many checkboxes from a list.

  • Radio Button: This module allows users to select a single radio button from a list.

  • Dropdown: This module allows users to select from a dropdown list that is created by the Template Builder.

  • Frameworks: This will allow you to embed a custom framework or rubric from your Sibme Account in a Form Template. For more on Frameworks in Forms, click here. For more on building Frameworks in your Sibme Account, click here.

  • File Picker: This module allows you to add a file to the Form Template OR require that users attach a file to the form when submitting. For more on the File Picker, click here.

  • Signature: This module allows users to type a name, which will be formatted in cursive writing in their form submission. You can also add a Disclaimer checkbox with custom text that you add to the format, and which users must click when adding their signature.

  • Section: This module allows the Form Builder to create pages in the Form Template. For more on Sections, click here.

  • User Field: This module allows you to choose data that is already entered in Sibme User Settings for the user submitting the form. For more on User Field, click here

  • User Data: This module allows the Form Builder to choose data that is already entered in Sibme User Settings for a different user from the user submitting the form. For more on User Data , click here.

  • Related Fields: This module allows you to link up to three different Form Modules together in a single field and then collect multiple submissions of the three fields. For more information on Related Fields, click here.
